Go to this site and scroll down, insert your Ethereum address (Public Key) and click on Submit.

Click 1. Auto generate key to generate a new EOS keypair.

2. Save your EOS Private Key. Best practice is saving your keys offline on a paper or an offline encrypted USB drive.

Important: You can do this lostkey process only one time! If you lose this private key, you will lose access to your EOS Account!

Then click on Alternate Signing - Ledger/Trezor.

Note: The alternative method is more complicated. If you have a problem with the alternative method, you can follow the guide here on connecting your hardware wallet to Metamask to use the simple method with Metamask.

Copy the text message 1. and click on 2. the MyCrypto.com link.

Login to mycrypto with your hardware wallet.

Here is a how-to guide that explains how to sign a message with mycrypto.com:

In our case, it should look like this:

Go to this site

1. Sign Message, insert the text message you copied under Message

2. Click Sign message

3., then click on 4. Verify message if it's not automatically changing after signing:

Then copy the 5, the signature message.

Go back to EOS Authority and enter the Signature message and click Submit signed message.

Troubleshoot: If you do not see the message in this format, try to sign the message with MyEtherWallet:https://www.myetherwallet.com/wallet/sign

If you see: "Error after signing: YOUR SIGNAURE MESSAGE SHOULD START WITH { AND END WITH } OR IT SHOULD JUST BE THE SIGNATURE ALONE" or if someone struggles to sign the lostkey message successfully with a hardware wallet, it could be that they are signing a message which is in hex rather than text.

We recommend using the old legacy version of MEW here: https://v5.myetherwallet.com/interface/sign-message

If all is done correctly, then you wait for 30 days. After 30 days, the EOS keypair to your Account will change to the new keypair you entered.

Save your EOS Private Key! The best practice is to save private keys offline on paper or encrypted on a USB drive.
